Booklist form working again

My apologies, but I just realized this morning, that the "add a book" form for the "Member Book List has not been working for several weeks now. I corrected the error early this morning.

If you've submitted a book for the list, and you don't see it up there, please accept my apologies and let me ask that you submit it again.
